NRD Request for Refund Form Revised
The NRD Request for Refund Form has been revised to reflect changes in the needs of firms seeking a refund.
Instructions and terms expanded.
The instructions and terms heading up the form have been expanded to address multi-jurisdictional refunds. They have also been modified to address refunds on an ongoing basis, and no longer refer to the October 17, 2003 “deadline” indicated on the previous form.
Section B: Request for Refund Schedule revised.
Section B of the form has been modified to allow for simpler completion of the refund information table. Please note that although only one table need now be completed, each refund type needs to be identified as per the table instructions.
